This privacy policy describes how Uniprix Zakrzewski-Jakubiak, Chilakos et Byer pharmacy ("we", "our pharmacy") collects, uses, and protects your personal information when you use this website or contact us through our online forms. We take your privacy seriously and comply with Quebec's Law 25 (Act to modernize legislative provisions as regards the protection of personal information) as well as applicable Canadian laws.

What information we collect

We collect only the personal information necessary to handle your requests. This information may include:

  • Your full name
  • Your contact information (phone number, email address if provided)
  • Your date of birth (for prescription transfer and renewal requests, to verify your file with previous providers)
  • The content of fields you fill in yourself on our forms (for example: prescription numbers, medications to renew, reasons for a consultation request, notes on a transfer request)
  • Automatic technical information: IP address, browser type, form submission timestamps, retained for security and fraud prevention purposes

How we collect this information

We collect the personal information you provide directly to us through the forms on our website (prescription transfer request, renewal request, new prescription drop-off, appointment booking for a service). We do not collect any personal information without your knowledge. Technical information (IP address, browser) is collected automatically when you submit a form, as is standard practice for website security.

Why we collect this information

We collect your personal information for the following purposes:

  • To process your specific request (file transfer, renewal, new prescription, service appointment)
  • To contact you to confirm a request, schedule an appointment, or ask for clarifications
  • To maintain a record of requests received, as required by our professional pharmaceutical duty of care
  • To detect and prevent abusive or fraudulent use of the site (for example: repeated submissions by a bot)
  • To comply with our legal and professional obligations, including those imposed by the Ordre des pharmaciens du Québec

Legal basis for collection

The collection of your personal information is based on your explicit consent, given each time you submit a form by ticking the consent checkbox. You may withdraw your consent at any time by contacting our Privacy Officer.

How long we keep your information

Your personal information is retained for 24 months from your last interaction with us (last form submission, last appointment, last call), then securely deleted. This duration is conservative and accounts for the possible need to verify the history of a transfer file or renewal request. Professional records related to prescribed care or pharmaceutical acts may be retained longer if required by law.

Who has access internally

Access to your personal information within our pharmacy is limited to members of the pharmaceutical team and authorized administrative personnel who need it to process your request. All employees are bound by strict confidentiality obligations as part of their duties, and pharmaceutical staff are further bound by the Code of ethics of the Ordre des pharmaciens du Québec.

Third-party processors

To provide our services, we use third-party providers. We do not sell or rent your personal information for commercial purposes. These providers are selected on the basis of contractual commitments to security and confidentiality equivalent to Quebec standards, and their practices are subject to periodic evaluations on our part. The categories of providers are:

  • Web hosting
  • Database hosting
  • Transactional email delivery
  • Appointment booking platform (planned)

Transfers outside Quebec

Some of the third-party providers we use process data on servers located in Canada (notably in Quebec and Ontario), and others on servers located in the United States. These transfers are necessary to provide our services and are governed by contractual agreements offering a level of protection equivalent to the requirements of Law 25. We have conducted a Privacy Impact Assessment before communicating your information outside Quebec, as required by law.

Security measures

We implement the following measures to protect your personal information:

  • Encryption of communications between your browser and our site (TLS / HTTPS)
  • Encryption of data at rest on our providers' servers
  • Restricted access to authorized team members
  • Submission logging (date, time, IP address) for security purposes
  • Strong passwords and two-factor authentication for administrative accounts

Your rights

Under Law 25 and other applicable laws, you have the following rights regarding your personal information:

  • Right of access: obtain a copy of the information we hold about you
  • Right of rectification: have inaccurate or incomplete information corrected
  • Right to de-indexation and to discontinuation of dissemination (where applicable)
  • Right to deletion of your information once the purpose of collection is fulfilled
  • Right to withdraw your consent at any time
  • Right to file a complaint with the Commission d'accès à l'information du Québec if you believe your rights have not been respected

How to exercise your rights

To exercise any of your rights or for any question regarding the protection of your personal information, contact our Privacy Officer by email (samson.uniprix@gmail.com) or by phone at (450) 689-8181. We will respond to your request within a reasonable time, generally within 30 days of receipt. If you are not satisfied with our response, you may file a complaint with the Commission d'accès à l'information du Québec (cai.gouv.qc.ca).

Cookies and analytics

Our site does not use tracking cookies. We use an anonymous analytics service that places no cookies in your browser and does not identify individual users. This service only allows us to measure the number of visits and the most viewed pages, for site improvement purposes. None of the information collected this way can identify you.

Children's information

Our site is not directed at children under 14 and we do not knowingly collect personal information from them. If you are a parent or guardian and believe a child under 14 has provided us with information without your consent, contact our Privacy Officer, and we will promptly delete that information. For pharmaceutical care of children, we always require parent or guardian consent, in accordance with professional requirements.

Breach notification

In the event of a confidentiality incident presenting a serious risk of harm to affected individuals, we commit to: (1) immediately taking the necessary measures to limit the incident; (2) notifying the Commission d'accès à l'information du Québec as soon as possible, in accordance with section 63.7 of Law 25; (3) directly notifying the individuals whose information was affected, by the most effective means (email or phone). We also maintain an internal log of all confidentiality incidents, whether or not they present a serious risk.

Changes to this policy

This policy may be modified from time to time to reflect changes in our practices or legislative developments. The current version is always indicated at the top of this page with its effective date. We invite you to consult this page periodically. Substantial modifications will, where possible, be communicated on the home page of the site.
